Please read carefully<\/p><p>Our practice collects and maintains a record of the health care services we provide you. In keeping with the Health Insurance Portability and Accountability Act (HIPAA), and current state regulations we are dedicated to protect your rights of privacy and the confidential information entrusted to us.<\/p><p>The commitment of each employee to ensure that your health information is never compromised is a principal concept of our practice. We will not disclose your protected health information unless you direct or authorize us to do so or unless it is otherwise allowed or compelled by law. We may, from time to time, amend our privacy policies and practices but will always inform you of any changes that might affect your rights.<br \/>You may see your record or get more information about it at \u201cYour Individual Rights about Patient Health<br \/>Information\u201d section of the Notice. Be informed that some regulations, such as those addressed under Substance Use Disorder rules are more restrictive than HIPAA rules.<\/p><p style=\"text-align: center;\"><b>PROTECTED HEALTHCARE INFORMATION (PHI)<\/b><\/p><p style=\"text-align: left;\">It is important that you know not only that we limit requests for your personal information to that needed<br \/>to provide quality health care, implement payment activities, and conduct normal health practice<br \/>operations, but understand what \u201cProtected Healthcare Information\u201d is. This may include your name,<br \/>address, telephone number(s), Social Security Number, employment data, dental history, health records,<br \/>and\/or any personal information that is unique to you.<br \/>While most of the information will be collected from you, we may obtain information from third parties if it<br \/>is deemed necessary. Regardless of the source, your personal information will always be protected to the<br \/>full extent of the law.<\/p><p style=\"text-align: center;\"><strong>PROTECTING YOUR PERSONAL HEALTHCARE INFORMATION<\/strong><\/p><p>We use and disclose the information we collect from you only as allowed by the HIPAA and the state regulations. Such instances may include:<br \/>1. Preparation of claims for payment of services,<br \/>2. Billing your insurance directly, including information that identifies you, as well as your diagnosis, the procedures performed, and supplies used so that we can be paid for the treatment provided,<br \/>3. Collection activities (if necessary) to obtain payment for services.<br \/><strong>Health Care Operations:<\/strong> We may use and disclose your health information to support the daily activities related to health care.<\/p><p><strong>Examples include:<\/strong><br \/>1. All such entities must sign<br \/>a Business Associate Agreement to protect the confidentiality of your private information.<\/p><p style=\"text-align: center;\"><b>ADDITIONAL USES AND DISCLOSURES<\/b><\/p><p>We also use and disclose your information to enhance health care services, protect patient safety, safeguard public health, ensure that our facilities and staff comply with government and accreditation standards, and when otherwise compelled or allowed by law not in conflict with exceptions established under HIPAA\u2019s Substance Use Disorder requirements. About FDA-regulated drugs and devices to the U.S. Food and Drug Administration.<\/p><p><span style=\"font-style: inherit; font-weight: inherit;\">2. To government oversight agencies with data for health oversight activities such as auditing or <\/span>licensure.<br \/>3. To public health authorities with information on communicable diseases and vital records.<br \/>4. To your employer, findings relating to the evaluation of work-related illnesses or injuries.<br \/>5. To workers\u2019 compensation agencies and self-insured employers for work-related illness or injuries.<br \/>6. To appropriate government agencies when we suspect abuse or neglect.<br \/>7. Their family spends a lot of time outdoors hiking, swimming, and camping in the beautiful Pacific Northwest. In 1999 Ilya moved to Seattle and worked in hospital<br \/>setting, but the passion toward dentistry brought Ilya to Dental Hygiene School,\u00a0where he completed his RDH degree.<br \/>Ilya works for Cascade Dental Group since 2018. He enjoys hygiene part of dental\u00a0care and passionate about sustainable gum health and good oral habits\u00a0education. He is a creative and efficient, Ilya keeps us smiling in the office with his<br \/>wit and humor.<br \/>Ilya is the director of Chameleon Theater. As a thespian, he writes, directs and\u00a0acts, in theater as well as small independent movie productions that he has\u00a0written and acted in himself! She worked with kids in a daycare, then decided to\u00a0pursue a career in the dental field.<br \/>Kristina completed on the job training in dental office for 3 months to earn her\u00a0Dental Assistant Registration (RDA). In May 2022 she started here at Cascade<br \/>Dental Group, Kristina is eager to give excellent patent care; she strives to make\u00a0patients feel comfortable and at ease. Dr. Kathy and Kristina have a good system<br \/>for 4 handed dentistry; while she takes every precaution and is extremely efficient\u00a0when sterilizing and cleaning instruments and equipment. Kristina likes to make<br \/>temporary crowns and in interested in orthodontics.<br \/>In her free time Kristina likes to travel with her husband.
